Power Over Pain (POP) is a collaboration between The Hospital for Sick Children (SickKids) and University Health Network (UHN)

SickKids, affiliated with the University of Toronto, is Canada’s most research-intensive hospital and the largest centre dedicated to improving children’s health in the country. B. END USER LICENSE AGREEMENT
Please read the following End User License Agreement (“EULA”), including all disclaimers and terms and conditions of use below, carefully before using the Portal. By using or accessing this Portal, you acknowledge that you have read, understood and accepted the disclaimers and terms and conditions of use contained in this EULA. If you do not accept any of these disclaimers or terms and conditions, you must not use the Portal.

Information in this Portal was been developed by SickKids and UHN.

Computer devices may malfunction from time to time and any unsaved data may be lost.  


3.    Governing Law and Jurisdiction. This EULA shall be interpreted under, and any action or claim arising from or related to the Portal shall be brought in the courts of, and governed exclusively by, the laws of Ontario, Canada and the applicable laws of Canada applicable therein, without regard to its conflicts of laws principles.  Unless prohibited by applicable law, you expressly waive the right to participate in a class action proceeding.


4.    Changes and Updates. SickKids and UHN reserve the right to update or change information contained in the Portal without prior notice or prior acceptance. 


5.    Purpose of Portal. The Portal is a virtual platform, co-designed by a diverse group of Canadian youth with chronic pain (CP) during the COVID-19 pandemic. The goal of this project is to enable youth living with chronic pain access to virtual resources through a Stepped Care approach. The Stepped Care model is a resiliency-based approach that favors providing rapid access to the least intensive intervention that is tailored to the person's needs, preferences, and readiness to engage in behavioral change. The intended solution is to create a portal where youth living with pain can access key interventions and resources that are most appropriate for them. The three main components of this Portal include [1] capturing of demographic and chronic pain information via self-report from users, [2] the completion of self-assessments using validated and reliable PROMIS Pediatric Pain Profile, and [3] providing evidence-based virtual care interventions for pediatric chronic pain as personalized resource. The Portal is not a medical device. Nothing contained in the Portal should be construed, directly or indirectly, as the practice of medicine or providing medical advice or medical services by SickKids or UHN. The Portal is not intended to diagnose, cure, mitigate, treat, or prevent disease or other conditions.


6.    Not Medical Advice. The Portal is not a substitute for professional medical advice, diagnosis or treatment. Never make changes to your medication, nor adjust your dose, without first consulting your health care provider. Always seek the advice of a physician or other qualified healthcare provider concerning questions you have regarding a medical condition, and before starting, stopping or modifying any treatment or medication. Never delay obtaining medical advice or disregard medical advice because of something you have or have not read on the Portal. If you have, or suspect you have, a health problem, or if you experience an adverse side effect, please consult your doctor. If you have, or suspect you are experiencing a health emergency, please call 911 and/or promptly visit a Hospital Emergency Department in your area. 


7.    No Regulatory Approval. The Portal has not been evaluated by Health Canada, the United States Food and Drug Administration (“USFDA”) or any other domestic or foreign regulatory agency and is not approved to diagnose, treat, cure or prevent any disease or for the prescription of any medication or treatment.  While the information contained may be of use in other jurisdictions, these materials were provided and designed for use in Canada and you are solely responsible for ensuring suitability and permitted use in other jurisdictions.

9.    Express Consent to Receiving Electronic Messages and Upgrades from SickKids and UHN. By using the Portal, you hereby expressly consent to receiving electronic messages from SickKids and UHN, including alerts and reminders, information, notifications, requests for your feedback, and other communications from SickKids and UHN and as well as any applicable software updates or upgrades.
 
C. COLLECTION OF DATA
1.    Transmission of non-identifying data. The Portal collects non-identifiable personal data (which is transmitted back to SickKids and/or UHN) through the Portal regarding the use of the Portal in order to better understand use of the Portal and to help SickKids and/or UHN identify areas for improvement. If you do not consent to the transmission of this data, then you must not use the Portal.


2.    Transmission and Storage of Identifying Data and PHI. The Portal also collects the user’s personal information and/or personal health information (“PHI”) including name (first name or pseudonym), province or territory (optional), email address, year and month of birth (optional), cell phone number (optional), and self-reported symptoms. This information is stored on the Portal and also transferred to the POP server. Information stored on the POP Server is accessible by authorized administrators at these sites. More information can be found in the POP Privacy Policy, which is accessible below.


3.    DISCLAIMER. THE PORTAL TRANSMITS YOUR PERSONAL INFORMATION AND PERSONAL HEALTH INFORMATION OVER THE INTERNET. INFORMATION TRANSMITTED OVER THE INTERNET MAY BE AT RISK OF INADVERTENT LOSS OR THEFT. SICKKIDS AND UHN CANNOT GUARANTEE THE SECURITY OF ANY INFORMATION TRANSMITTED TO OR FROM THE PORTAL. BY USING THE PORTAL, YOU AGREE THAT YOUR ACCESS AND USE OF THE PORTAL AND THE CONTENT, INCLUDING BUT NOT LIMITED TO PERSONAL INFORMATION AND PERSONAL HEALTH INFORMATION, IS ENTIRELY AT YOUR OWN RISK AND LIABILITY.
 
D. LIMITED LICENSE.

Through your website access, provided that you comply with the terms of this EULA, you are acquiring, and UHN grants you a personal, limited, non-exclusive and non-transferable license to use the Portal on your device for personal, non-commercial use and subject to any usage rules set forth by the distributor. This EULA does not entitle you to receive from UHN hard-copy documentation, support, telephone assistance or enhancements or updates to the Portal.

Privacy policy

This privacy policy governs your use of the software application Power Over Pain (“POP”) that was created by the Hospital for Sick Children (SickKids) and the University Health Network (UHN), Centre for Digital Therapeutics. The Centre for Digital Therapeutics is a digital health research and development group that is hosted at UHN.

The POP portal is a virtual platform, co-designed by a diverse group of Canadian youth with chronic pain (CP) during the COVID-19 pandemic. The goal of this project is to enable youth living with chronic pain access to virtual resources through a Stepped Care approach. There will be no alerts to healthcare professionals within the portal. It is not intended to replace your current care, but to supplement it with additional support. 

Our Commitment to Privacy

This Privacy Policy provides you with information on how the POP portal collects, manages, and protects your data (including any Personal Health Information) which you share within the portal. It further describes which particular data may be shared with others, what data will not be shared, and how you are able to control and manage this sharing.

When you choose to create an account on the POP portal, you will be required to agree to this Privacy Policy and to the Terms and Conditions (presented separately). By agreeing to the Privacy Policy, you are providing consent for your Personal Health Information to be stored and used by POP as described below.

Data Collection & Usage

Creating your Account

If you agree to use the POP portal to complete your self-assessments, keep track of your trends, and access evidence-based resources, you will be required to provide your name and email address on the website. Your month and year of birth, province or territory, and phone number can be provided at your own will. This information is used to create your account on the portal.

Setting up your Account

Once you receive the registration email, a link will be provided to you that will allow you to activate your account. By clicking on the link, you will be redirected to the portal and asked to log in one final time. If you do not activate your account by clicking on the link in the registration email, you will not be permitted to use the portal.
Once your POP account is set-up and activated, you will be able to have access to all the portal features.

Completing Your Self-Assessment

With an objective to provide tailored and evidence-based self-management resources, you have the option to complete a self-assessment questionnaire which will prompt you to answer 24-32 self-reporting questions. The questionnaire will ask you to report on your pain, mood, worry, sleep, and substance use.

  • Why are we asking about mood?  Pain can lead to feeling down, low, and sad. This impacts your level of pain because negative moods and pain are processed in some of the same areas of the brain.

  • Why are we asking about worry? Any level of worry (like feeling anxious or nervous) can impact the level of pain you feel in your body. And pain can lead to more worries. Worrying involves many physical sensations and anxious thoughts, which in turn can amplify your pain. Your pain can also increase your worry.

  • Why are we asking about sleep? Pain can impact both how long you stay asleep and the quality of your sleep. Sleep is an important function which gives your body a chance to perform maintenance and recover from a tough day. Without good quality sleep, you may notice an increase in your pain and ability to cope with your pain.

  • Why are we asking about substance use? Some teens turn to substances to try and cope with their chronic pain. Unfortunately, any level of substance use can have negative impacts on chronic pain and related symptoms. Assess today to learn about substance use, and how this may be impacting your pain and function.

Your responses for the self-assessment will automatically be saved. This way, if you leave the page and return, you may resume your self-assessment. To ensure that the responses are still relevant at the later date/time at which you wish to resume the assessment, you will be directed to the first question for a chance to review all submitted answers and ensure they are still valid at the time of submission.
 
Self-Assessment Results

  1. The data collected in the self-assessment questionnaire will be used to generate a score and feed into an algorithm. This algorithm will inform:
    Your trends graph - which can be used to track progress over time. The graph can be used to see daily results, or trends over a range of time.

  2. Your results cards - which will provide you with your numeric score, a weather icon which metaphorically represents symptom status, along with a description of what your score means.

  3. Your tailored resource recommendations - which can best support you on your self-management journey.

Liking and Completing Resources

Users have the ability to like a resource, or mark it as complete. These features are intended to enhance the user experience to enable easier access to favourable resources. Users will be able to filter their resources based on their personalized liked and completed resources.

Automatically Collected Information

The Portal collects certain information from you automatically, which is limited to analytics around the Portal screens you use. This automatically collected data will only be used by the administrative team within UHN responsible for maintaining the Portal, and for hosting the data submitted to the Portal, and may include third party agents of UHN, acting solely on UHN’s behalf and will only be used for system administration purposes, where access to such data will be limited to diagnosing problems with servers (such as software bugs), if any; to monitor traffic patterns; to determine which services are used most frequently; to assess site usage; to help to improve the usability of the Portal.

By creating an account with the Portal, you are agreeing to the automatic collection of the data listed above, solely for use by the Portal Team.

In addition to the use of the Automatically Collected Data described above, the Portal Team may also use information provided by you in response to the patient-reported symptom survey that has been stripped of all directly identifying personal information (such as your name, email address and other information which directly identifies you) for the purposes of improving the quality of the Portal.
 
Cookies

The Portal may use “cookies” for the convenience of users and to enhance your experience using this Portal. A cookie is a small text file that is recorded either temporarily or persistently on your computer by a web page server and is not intended to deliver viruses or other harmful programs to your computer.

On the Portal, cookies are used primarily for authentication and security purposes:

  • Cookies are used to avoid signing into your account repeatedly with 1+ authentication within a period of 30 days. By using cookies, POP is able to deliver a more efficient service that is more personalized for each user.

Web browsers generally accept cookies automatically. When you sign in to the Portal, you can:

  • Opt for the Portal to remember you and then it will use cookies to do so
                 or

  • If you prefer, you may choose to set your web browser to decline cookies
                 or

  • You may be able to modify your browser setting to notify you each time a cookie is created and permit you to accept or decline cookies on an individual basis or to accept cookies from web sites that meet certain disclosure and use specifications.
    Note that some functionality of Portal may be limited if you direct your browser to reject all cookies.

How We Share Your Data

With Third Parties

We will not sell, rent, or share your Personal Health Information with third parties other than as outlined herein.

We may disclose your Personal Health Information as required by law, such as to comply with a subpoena, or similar legal process.

How You Can Control Your Data

You will have control over your Personal Health Information. If at any point you identify information that needs to be corrected or removed, you can contact the POP Team by email at popsupport@cdtx.ca 
 
How We Store Your Data

All information gathered from you will be stored and processed in Ontario, Canada on University Health Network servers. If you access your POP account from a location other than Canada, your information will be transferred to Canada. Ontario and Canadian law are different from that of other jurisdictions.  Ontario and Canadian laws apply to the use, storage, transfer, management and handling of your data.

The Portal may retain your PHI for as long as you use the application. You may submit a request with your provider to discontinue your participation in Portal, subsequently the collection of any new Personal Health Information in the application will be stopped. Historical Personal Health Information in Portal will be handled in accordance with this Privacy Policy.

Your records will be retained for at least ten years.

How We Keep Your Data Secure

The Portal has implemented security measures intended to protect your Personal Health Information both online and offline, including measures intended to restrict unauthorized persons from accessing Personal Health Information. Encryption technologies are used when transferring and receiving data provided through the Portal. We also have security measures in place at our physical facilities to protect against the loss, misuse, or alteration of Personal Health Information stored in the Portal. We also have restricted access to the servers to the Portal Team, and have the ability to audit who accesses your data and when. Although we have implemented various measures to protect your information, the Portal does not and cannot guarantee absolute security of any information.

We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. For example, we limit access to this information to authorized employees and contractors who need to know that information in order to operate, develop or improve our Portal. 
 
Theft, Loss or Unauthorized Access of Your Information
In the event that UHN becomes aware that your Personal Health Information submitted through the Portal has been stolen or lost, a person has obtained unauthorized access to it, or someone has used, disclosed or disposed of in in a manner other than as described in this policy, UHN will investigate the incident and notify you if a breach has occurred.  If you become aware of any unauthorized use of your Personal Health Information submitted through this Portal, please immediately notify UHN Privacy Office at the address below.
 
Modifications and Updates to the Privacy Policy
This Privacy Policy may be updated from time to time for any reason. Please also check this Privacy Policy regularly for any changes.

Applicable Law
This Privacy Policy governs your use of the Portal. This policy was created by the Centre for Digital Therapeutics, UHN in compliance with requirements of Ontario’s privacy legislation, the Personal Health Information Protection Act (PHIPA).  Without regard to conflicts of law principles, and exclusive jurisdiction and venue for any dispute arising out of this Privacy Policy, shall be the Provincial courts of Ontario or Federal courts of Canada.
